In order for us to be able to submit a request to the relevant Association, please can you complete the form under related documents on behalf of each player and return this to us (fax or email would be preferable) at your earliest convenience.
Please note that the other Association has up to 30 days to respond to our request, although we would normally hope for an answer must faster than this.
If, after 30 days we have still not received a response then we may issue a provisional clearance in accordance with FIFA Regulations, which will enable this player to play for you.
