Referee Registration Form
All sections of the Referee Registration Form must be completed as indicated with the exception of the Equal Opportunities Monitoring questions, which are optional.
The completed forms should be returned to your County FA by 31/06/2009 and NOT the FA, otherwise a late fine may be imposed.
